• Associate Faculty Employment Packet (201kb PDF* file) – All forms required for new associate faculty.  The I-9 form must be completed in person within three days of employment. Call Human Resources at 972-548-6659 with questions or for a list of individuals trained to complete the I-9 form for new employees. To access these forms, you must use Adobe Reader. Please print the forms, complete and return the entire packet to Jonene Kemp in Human Resources at Courtyard Center by the monthly payroll cut-off date.

Added for certain Associate Faculty effective September 2004:  Group insurance and flex plan eligibility for the State of Texas Group Benefits Program through ERS, under current State law, if you taught at least three credit hours or one credit course each of the last six long semesters (Fall and Spring) over the last three years, AND are scheduled to teach at least twelve hours at some point during the academic year.  Associate faculty are not eligible for a State contribution or local/ employer contribution toward health insurance premiums or any other coverage under the State's plan and funding is not available at this time to help share the health premium cost as for full-time and part-time TRS-eligible employees.  For more details about the current offerings per State law, please see the Associate Faculty Insurance/TexFlex Benefits Summary.

• Continuing Education Employment Packet (185 kb PDF* file) – All forms required for continuing education instructors.  The I-9 form must be completed in person within three days of employment.  Call Human Resources at 972-548-6659 with questions and for a list of individuals trained to complete the I-9 form for new employees. To access these forms, you must use Adobe Reader. Please print the forms, complete and return the entire packet to Jonene Kemp in Human Resources at Courtyard Center by the monthly payroll cut-off date.  
   

• Part-time Non-Teaching Employment Packet (221 kb PDF* file) – All forms required for new part-time staff employees.  The I-9 form must be completed in person within three days of employment.  Call Human Resources at 972-548-6659 with questions and for a list of individuals trained to complete the I-9 form for new employees. To access these forms, you must use Adobe Reader. Please print the forms, complete and return the entire packet to Mae Francis in Human Resources at Central Park Campus, room A107 by the monthly payroll cut-off date.

• Part-time Application for Employment (Continuing Education, Fire Science, Law Enforcement Associate Faculty and Camp Employees ONLY)

• CHANGE FORM ( Banner Master Change Form) - use to update campus location, marital status, home address/phone/email, and SSN.  Submit completed form to Human Resources for processing.
